Reentry Medical Case Management

Reentry Medical Case Management is a UNIFIED program to assist individuals living with HIV who are currently incarcerated or as they transition back into the community post incarceration to ensure they are linked to and remain in care. 

Our client centered approach works with individuals to remove barriers and optimize health outcomes. Our caring team of reentry medical case managers work with individuals to facilitate access to medical care, treatment, insurance navigation, dental and optical care, behavioral health services, substance use disorder treatment, housing, food, reentry specific support group, transportation, and other social support.

This program serves Lapeer, Macomb, Monroe, Oakland, St. Clair and Wayne Counties and provides centralized intake for all Michgian Department of Corrections (MDOC) facilities.
