Essential Duties and Responsibilities
  • Provide continuum of care services to persons with HIV/AIDS disease including but not limited to: intake, referral, assessment, coordination, monitoring, reassessment, and follow-up.
  • Monitor proper documentation of client services, and help Case Manager to improve documentation.
  • Meet bi-weekly with each assigned medical case manager to review client cases for accuracy and adherence to agency standards
  • Assist Director of Care Services in collection and analysis of data
  • Ensure compliance with the Principles and Standards for Service for HIV/AIDS Case Management in Michigan as well as the Southeastern Michigan HIV/AIDS Council Ryan White Part A Care Standardsand Ryan White Part B Standards.
  • Provide case management services in a manner that meets culturally and linguistically Appropriate Standards (CLAS) of the organization.
  • Track and report all case management activities through the CAREWare system.
  • Participate in quality improvement and assurance efforts through chart reviews, regular data collection and measurement related to quality indicators.
  • Ensure optimal client care by participating in individual and team supervision.
  • Work with Director of Care Services and Vice President of Programs and Operations to resolve any personnel issues.
  • Assists in the hiring process for new staff for the program.
  • Transport clients to medical appointments in personal vehicle.
Supervisory Responsibilities

This job supervises the Medical Case Management and Henry Ford Care Coordination Staff.

Education and/or Experience

Bachelor’s degree in social work, psychology, nursing or a related field and a minimum of 2-3 years of experience in health care, mental health or substance abuse preferred.  Experience and/or training related to HIV/AIDS, substance abuse disorders, mental health and homelessness preferred. Must be able to function independently and be able to work well with individuals from a variety of socioeconomic and multi-ethnic groups.  Must be culturally sensitive to the LGBTQ community. Familiarity with evaluation and quality assurance in case management services.  

Must have insured personal transportation to be used in key functions of the position.
